EC Records Request Forms

Clay County Schools Exceptional Children Department requires 10 business days for record retrieval. Upon receipt of a completed Record Request Form, we will retrieve the requested documents as soon as possible.

 

If you are requesting a record bot not the student and the student is over 18, some form of guardianship documentation must accompany the request to allow us to provide you a copy.

 

Once the record is printed, it is available for pick up at the Clay County Schools Central Office located at 154 Yellow Jacket Drive, Hayesville, NC  28904.

 

A record can be emailed or faxed if that is designated on the Record Request form.
